Preparing my Initial Application for SSD

Before you fill out your intial application for Social Security disability benefits you should know what they are going to ask you!

The Social Security Administration is going to want to know the names and addresses of the employers you have worked for in the last 15 years before you became disabled. They will also want a description of your job duties. It is crucial that you are accurate about this information, particularly your job responsibilities and duties. Don’t embellish! Explain your job duties as you performed there, including any supervisory responsibilities.

Social Security Administration will also want to know the doctors, hospitals, clinics, and other medical providers that you treated with or have been evaluated by, together with the addresses and dates of treatment.
